During the time the families
are assisted under DHAP, each family must participate
in case management services provided by the HCHA
through its Social Service Agency contractors.
HCHA will report case management outputs and resident
outcomes through a HUD web-based tracking system.
The system will track information such as the
number of families being served and progress toward
self-sufficiency goals.
HCHA selected several social service agencies
to conduct case management services for DHAP Families
and coordinate with social service agencies that
previously provided case management services to
DHAP Families since the 2005 hurricanes. HCHA
also contracted with a Project Manager to coordinate
the social service agency contractors for better
effectiveness and efficiencies.

DHAP Intake Process
The initial case management contact with the DHAP Families will take place during the
DHAP Family Briefing that will be conducted by the DHAP housing administration team.
Each family will be scheduled to attend a briefing where they will receive instructions concerning the transition from the FEMA housing program to the new HUD Disaster Housing Assistance Program. At this time, the DHAP Families will be provided program responsibilities as outlined by HUD, which will include information concerning the mandatory social service case management aspect of the program. During this intake process, each DHAP Family will be required to execute a case management preliminary assessment form (social service intake form) that will request information concerning:

Needs Assessment
The DHAP Intake process will also include a Needs
Assessment in order to determine the type(s) and
estimated quantity of social services needed.
Households will be assigned to "tiers" of case
management based on the intensity of services

Individual Development Plans (IDP)
Based on the DHAP Family’s specific needs assessment
findings, the Case Manager, in conjunction with
the DHAP Family, will develop an Individual Development
Plan.
The IDP:
Cites the activities, services, objective
& measurable interim and final goals
Documents consistent prescribed policies
and procedures
Provides a roadmap to individual self-sufficiency
The case managers will track the DHAP Familys
IDP progress by conducting tier-specific interval
contact with the family to gather documentation
and address issues, concerns or perceived or real
roadblocks that may hinder the Familys attainment
o economic self-sufficiency through the program.
This information will be tracked and reporting to
HCHA on a monthly basis and HUD quarterly. See more
